To add a gift tick box to an appeal:
Click Settings.
Select Set-Up, then General.
Scroll down to Basket Donation Appeal.
Select the relevant appeal.
Update the settings as required.

You can include a gift tick box for an appeal by updating your settings, allowing supporters to add a gift option when donating to that appeal.
